Medical Authorization
In the event of illness, injury, accident, or medical emergency during participation in the event, I authorize the Event Director and their designees to obtain emergency medical treatment deemed reasonably necessary.
For participants under 18 years of age, I authorize emergency medical treatment for the minor participant if a parent or legal guardian cannot be immediately contacted.
I understand and agree that:
Emergency medical services may be provided at my expense.
